Conference Interview Form – This is the basic interview evaluation form which should be used for first-time license interviews and finalization interviews.
Conference Interview Form – 3 year cycle interview – The COMS may use this form when interviewing license-holders for their 3-year update interview.
Conference Interview Form for Reactivation – This interview form should be used for those seeking reactivation of their permanent credential.
Serve Globally Interview Form – This interview form is to be used by the Serve Globally COMS for all credentialing interviews.
Sermon Evaluation Form – This form is a resource to assist you in evaluating sermons submitted by ordination candidates.

Sample interview questions regarding women in ministry – These questions were developed by the board of Advocates for Covenant Clergy Women (ACCW).
